Birth Certificate

"Notifications of Birth Registration" or "Birth Announcements," which do not meet the requirements outlined below, are not acceptable as evidence of citizenship.

For a Birth Certificate to be acceptable, it must:

  • Be issued by the office of vital statistics of the state, county, or city where your birth occurred.
  • Show your full name at birth.
  • Indicate the date and place of your birth.
  • Bear the embossed, impressed, multi-colored, or raised seal and signature of the issuing authority; and indicate a registration/file date within
    one year of your birth.

 

 

ACCEPTABLE PASSPORT PHOTOS

You must go to a passport photographer who knows the requirements and has a special passport camera that shoots two identical photos simultaneously. You can have passport photos made at a Kinko's, photo stores or look in your local yellow pages under "passport photos". The photos must meet the following requirements:

1)   2X2 inches
2)   Pale color or white background
3)   Full face, front view
4)   The photos must be identical
5)   The photos must have been taken within the last six months
6)   Photographs retouched so that your image is changed are unacceptable
7)   Do not wear a hat unless for religious reasons (if so, please include and explanation)
8)   Wear normal day-to-day attire. Do not wear uniforms
9)   If you normally wear a wig, hearing aid or prescription glasses, wear them for the photo
